Adults Physical Disabilities Social Worker
Role Purpose
To support adults with physical disabilities to live safely, independently, and with dignity by assessing their needs, coordinating support, advocating on their behalf, and ensuring they receive appropriate services in line with legislation and local policy.
Key Responsibilities
Assessment & Planning
- Undertake strength‑based assessments under the Care Act 2014.
- Identify eligible needs and develop personalised care and support plans.
- Carry out risk assessments, mental capacity assessments, and best-interest decisions where required.
- Review care plans regularly to ensure they remain appropriate and effective.
Case Management
- Manage a caseload of adults with physical disabilities, including long‑term conditions, acquired injuries, and progressive illnesses.
- Coordinate multi‑agency involvement (OTs, NHS teams, housing, community providers).
- Record and maintain accurate case notes on appropriate systems.
Safeguarding
- Identify, investigate, and respond to safeguarding concerns in accordance with statutory duties.
- Apply Making Safeguarding Personal principles.
- Develop protection plans to ensure safety and minimise risks.
