Contract: Temporary - 3 Months

This post will be providing Team Manager cover for the PD North Team while the Manager is off work due to sickness. Funding has been agreed for 12 weeks.

Key Roles and Responsibilities To lead and manage The PD North Team, ensuring the delivery of high-quality, person-centred social care services that promote independence, safeguard vulnerable adults, meet statutory duties, and achieve positive outcomes for individuals with physical disabilities and their families. The team is very virtual currently and only has a monthly team meeting as standard office attendance. This meeting is usually in Basingstoke.

1. Team Leadership and Management

Provide day-to-day leadership, supervision, and support to social workers, and case workers/senior case workers

Manage staff performance, attendance, workload allocation, and professional development.

Undertake regular supervision sessions

Promote a positive team culture and staff wellbeing.

2. Safeguarding and Risk Management Responsibilities

Ensure safeguarding concerns are managed effectively and in line with statutory guidance.

Oversee Section 42 safeguarding enquiries where appropriate and chair safeguarding meetings.

Make decisions regarding risk management and protection planning.

Escalate serious concerns (including through Risk Form and Risk Escaltion Panel)

3. Case Management Oversight

Monitor the quality and timeliness of assessments, reviews, and support plans.

Ensure interventions are person-centred, strengths-based, and outcome-focused.

Provide guidance on complex cases and high-risk situations.

Ensure all funding requests are considered through SECAF/SPOG process

4. Care Act and Statutory Duties

Ensure compliance with:

Care Act 2014

Mental Capacity Act 2005

Equality Act 2010

Human Rights Act 1998

Oversee eligibility determinations and care planning processes.

Ensure the Hampshire County Councill meets its statutory responsibilities.

5. Financial and Resource Management

Manage team budgets and oversee expenditure on care packages through the SECAF/SPOG process.

Ensure services provide value for money while meeting assessed needs.

Support demand management and service sustainability.

6. Partnership Working

Develop effective relationships with:

NHS services

Voluntary organisations

Advocacy services

Independent care providers

Represent the service at multi-agency meetings and strategic forums.

7. Quality Assurance and Governance

Audit case records and undertake Quality Assurance Forms (QAFs)

Ensure compliance with local policies and procedures.

Respond to complaints, serious incidents, and service reviews.

Support service improvement initiatives

8. Supporting Independence and Inclusion

Promote independent living and community participation.

Ensure individuals have appropriate referrals for equipment, adaptations, and support.

Encourage strengths-based approaches.

Support transitions and long-term planning.

9. Strategic and Service Development

Contribute to service planning and transformation programmes.

Implement new legislation, policies, and operational procedures.

Use performance data to improve outcomes and service delivery.

Lead change management within the team.
